Premaxillary Ridge Reconstruction: The Story Beyond GBR!!

Management of advanced premaxillary atrophy poses as a complex reconstructive challenge in implant dentistry and maxillofacial surgery, particularly when horizontal and/or vertical deficits preclude ideal implant positioning. Bone augmentation using allogenic block grafting remains the benchmark for volumetric hard tissue reconstruction in these cases, offering superior structural stability and biological integration compared to particulate GBR alone. This surgical protocol involves full-thickness flap elevation, recipient site decortication, allogenic bone grafting, and rigid fixation with titanium screws. The use of adjunctive particulate grafts and resorbable barrier membranes is integrated into the protocol to enhance marginal stability and graft consolidation. Healing is assessed via clinical evaluation and CBCT imaging over a 4month postoperative interval prior tostaged implant placement.This technique requires advanced surgical skills and careful flap management to mitigate risk of exposure and graft loss.
