The management of the edentulous and terminal dentition patient increasingly relies on the convergence of three-dimensional diagnostics, prosthetically driven implant planning, additive manufacturing and digitally mediated prosthetic rehabilitation. In this frame, stackable guided implant surgery has emerged as an advanced methodology designed to establish a controlled relationship between the planned prosthesis, alveolar bone modification, implant positioning, and immediate provisional restoration. In this surgical session, Dr Shankar Iyer will critically examine stackable guide technology beyond its conventional description as a sequence of surgical templates, focusing on the principles of digital treatment transfer, biomechanical optimization, and prosthetic surgical congruence. The prosthetic implications of stackable guided surgery and Implant placement have been evaluated well in consideration with restorative emergence, cantilever biomechanics and long-term peri-implant tissue stability. The relationship between the accuracy of implant positioning and the predictability of immediate provisionalization have been critically explored. Importantly, the limitations of stackable guide protocols will be addressed in detail by the speaker, Dr Shankar Iyer including inadequate fixation, cumulative guide-transfer errors and the mismatch between surgical and prosthetic reference systems. This presentation ultimately positions stackable guided surgery as a complete digital treatment protocol rather than a mere guide manufacturing technique.
